my 2p:

i'd clone out the necklace protruding from his mouth, looks like he is dribbling ;]

crop it so you lose that arm coming in from the right hand side as well

when composing a photo like this, I would look to try and move so you don't have people in the background like this one
of course this isn't always possible depending on how crowded the beach is

Lovely photo. Love the warm light. Would crop off the sky, a bit distracting to me.
