Medicine Show, What do I do next?

I'm developing this image for the environment part of the Artstation Wild West challenge. I've never really tried to incorporate 3D into my workflow like this before, and honestly I'm stuck. I know this image needs to be improved but I don't really know where to take it from here. Any advice?

You could always add a bit of story to the image. A couple of examples:

In the first image he's been hanged for being a charlatan. In the second he's been hanged for witchcraft, but used one of his potions before-hand and survived the hanging.

The best way to hunt in a mushroom forest is to climb on the back of a butterfly.
