They all look real to me. Just some nifty camera tricks and a little bit of PS.
Seems they use this camera lens, which may add to the "fakeness" of the pics - Lensbaby (http://www.lensbabies.com/)

They're all real, just shot with a telephoto lens plus ps to artificially increase the DOF.

decrease? but yes theyre really nice. it used to be done only by large format cameras where you could do tilt and shift movements of the film planes vs the lens so that you could get this crazy shallow depth of field.
but yeah, instead of hauling a view camera along, you can now use almost any camera and ps to this.

Magic Man is right, it's a tilt shift lens, although this effect is fairly easy to reproduce using masks and blurring in Photoshop, as Elwell said, someone posted a tutorial on it... if you don't wanna go out and buy a lens :)
Nice effect though. Although I've seen it being used a lot more recently, it's getting a bit trendy... down boy!

You could also fake that with Photoshop- Use a mask gradient to select a gradient bar horizontally in the middle of the photo. Invert this so that only the top and the bottom are selected and there's a gradient leaving the middle area 'clear'.
Run photo blur filter over it (you could also use gaussian blur). Keep playing around with this until it looks right. I've done this myself a couple of times. It works best for aerial shots of buildings/trains/etc. Think about what a macro lens would photograph and find a similar photo subject. A macro lens works by having a very limited depth of field, so pick a subject and select a very specific area. You can also erase objects from the quick mask mode that are further away from the camera than the selected dof area.
