that's pretty wicked! i like the whole creepy grin thing.

Did he just do the carving into his cheeks? i'd think there would be a lot more blood. Also i'd suggest you change the angle of the knife he's holding, cause right now it can be confused for stabbing himself in the stomach.. but you never know what this guy'll do, huh

Also, there are some weird things going on with the angle of his face. His eyes are looking straight out as if it was a perfect frontal view, while you've got the angle of the ear and jaw turning more towards a 3/4 view. When the face turns, eye going away from the viewer gets closer to being lost behind the nose. take a look into that
