I'm about half and half for opacity on my brushes (I use all custom brushes). For regular drawing, I don't mess with opacity at all and just work with adjusting the flow rating. This is especially true for line work and base coloring.
Once I get to shading and adding shadows, I use large soft brushes where the opacity and flow are both linked to tablet pressure; it gives the best control over the heaviness of the shading I find.
As for emulating pencil lines, I use a gritty brush with a tight pattern I made myself and set opacity to 100% and flow to around 60 to 70% or so. It's not a perfect pencil emulation, but it retains the tone and overall feel of pencil lines.
