question about backgrounds

im just wondering how people create their backgrounds? do you start with a horizon and work from there? do you use a ruler? could someone describe the process for me?

I myself don't do backgrounds too often, but from what I've heard the best way to start is with the sky or light source. That way you can keep referencing it for how everthing else will be shaded.
